What else should you know?

  • To avoid having to track down patients for failed debit/credit card transactions, the first appointment fees are due at the time of service.
  • This plan is not insurance or a qualified plan under the Affordable Care Act.
  • Payment for restorative procedures not included in the plan are due at the time of service.
  • This plan does not cover procedures completed by anyone outside of this office.

How does payment work?

  • Option 1: Make the yearly payment at the first visit & get the restorative discount immediately - this option is required if patient has work done on the same day as signing up.
  • Option 2: Pay the office fees at the first visit & if you come back for routine care, need restorative treatment, or an emergency visit within 6 months, the first visit payment will be applied toward the balance remaining for the plan.
